Registration/Forms
Each time your child is NOT going to ride the bus home from an Away Sports Competition, you must fill out the form below and have it signed by the Western Middle School Athletic Director (Mr. Phil Tarantino) PRIOR to the start of the game.
FinalForms
The GPS Athletic Department has partnered with FinalForms, an online forms and data management service, to manage athletic participation. All new and returning student-athletes must register with a parent/guardian through FinalForms prior to tryouts or participation.
Click on the Parent Playbook for directions on how to register in FinalForms. If you require any support during the process, scroll to the page bottom in FinalForms and click “Use Support”.
Initial registration will take 15-20 minutes. Registering returning athletes can be completed in 5-10 minutes as FinalForms saves data season-to-season and year-to-year. Parents should have information for physician, dentist, emergency contacts, insurance (if applicable), and a current State of CT Health Assessment Record (HAR) form ready for the registration process. The HAR can be found on GPS Health Forms and Immunizations.
Please register early, as it will take 24-48 hours for the Health Office to process your registration. You may only register for one sport each season. There is no cost to register.
About the CT HAR FORM:
A student athlete must have a current State of CT Health Assessment Record (HAR) form on file with the Western Middle School Health Office prior to participation. For sports participation, this form is good for 13 months and must be valid at the time of tryouts and throughout the season. We are no longer using the separate Sports Physical form.

Intramural Sports
One of the main objectives of the Middle School Intramurals is to provide every student with the opportunity to play and enjoy friendly activity under the supervision of certified staff. While the main focus is recreational, participants may practice game skills learned in physical education class. Activities which are not offered in regular physical education class are also available. These offerings run for approximately 8 weeks as interest and participation dictate. All equipment is provided for the students and there is no cost to participate.
After School Intramural Sports are typically offered Monday, Tuesday, Thursday, and Friday.
Starting Time: 3:00pm until 4:00pm (late bus available on Monday, Tuesday and Thursday only)
(Students must be in the gym by 3:00 pm, and arrive with a pass if coming after 2:45 pm.)
Any student is eligible to participate: no tryouts or forms needed!
Transportation is provided through Western's late buses.
Flag football, Basketball, and fitness room will be offered daily!
